Photocopy the page(s) of your passport that contain your passport number, photo and signature.
- UK and International Passports are accepted

- Passports must be dated prior to connection
- Passport extensions and written passports are NOT accepted
- Expired/Out-of-Date Passports are NOT accepted

- Old-style Driving Licences - photocopy of the front of the licence

Utility Bill Photocopy
Accepted:
- Water bill, gas bill, electricity bill, landline telephone bill, council tax bill
- Printed 'online' bills.
- Landline telephone bills must show your landline number
- Bills must be new (less than 12 weeks)
- Bills must be dated prior to the connection date of your mobile phone
- Printed 'online' bills - bill must have been mailed to your home address
Not Accepted:
- Mobile phone airtime bill
- Final demands, red letters or bills indicating late payments
- Bills older than 12 weeks
- Bills dated after your mobile phone connection date
- Photocopies with any information obscured

Bank Statement Photocopy
Must be a separate account to any used as your Proof of Signature
Accepted:
- Recent statements (less than 12 weeks)
- Printed 'online' statements
- Statements must dated prior to the connection date of your mobile phone
- Statements must show transactions or some other sign of activity
- If your account is in arrears, statements must show an agreed overdraft limit
- Printed 'online' statements - must have been mailed to your home address
Not Accepted:
- Statements older than 12 weeks
- Statements dated after your mobile phone connection date
- Printed mini-statement or over-the-counter Requests statements
- Statements for accounts in arrears or which show negative activity e.g. bounced cheques or failed standing orders
- Account summaries that show no transactions
- Photocopies with any information obscured

Sign Your Contract
Your phone package included a blank Network Contract (unless you are on the 'Three' network, in which case skip this step).
Sign the contract and put it in to the envelope with your proofs and voucher.
Send Them Back To Us
Double check that your Proofs are valid, drop them in the envelope with your Voucher and send this back to us by Recorded Delivery or Special Delivery within 90 days of your connection date to:

- Incorrect claims will delay your Gift.

What if I haven't got these Proofs?
If you find the proofs of signature or address difficult to come by you can send us your first two airtime bills along with your claim voucher. Your mobile number must be visible on both bills. Your second bill should show that the first bill is paid in full and you have not changed your tariff.
Can I send anything else?
No. If it is not explicitly listed above then we can't accept it.
